申請警察刑事紀錄證明書(Police Criminal Record Certificate)– 良民證 教學
警察刑事紀錄證明書，俗稱 良民證 ，為中華民國內政部警政署根據《警察刑事紀錄證明核發條例》相關規定，由人民提出申請，而由政府機關核發之文件。文件上記載警察機關依司法或軍法機關判決確定、執行之刑事案件資料等相關事項，證明文件所載之當事人沒有任何刑事犯罪紀錄。
準備要來加拿大打工度假/遊學/留學/移民的同學們，所需要辦理的手續及文件有：體檢證明、 良民證 、財力證明和保險證明等，已經抽中打工度假的或是準備要出發的同學快快開始行動去申請吧！

申辦方式：
臨櫃申辦
備妥應辦文件前往各縣市政府警察局外事科辦理，不受戶籍所在地限制。部分縣市警察局提供快速發證及授權其他受理單位等服務。

網路申辦
於內政部警政署警察刑事紀錄證明申辦作業系統填寫申請人資料送出後，於2.5個工作天後攜帶應備文件前往申請時所點選之領件地點驗證及繳費領件，保存期限為發證日起30日，逾期作廢。

手機APP申辦
應使用手機下載「警政服務APP」申辦，於送件完成後，於2.5個工作天後攜帶應備文件前往申請時所點選之領件地點驗證領件。
統一超商申請
申辦及領證地點僅限台北市及桃園市，於統一超商依照i-bon機指示輸入手機及身分證字號後列印申請書，於填寫完畢後掃描回傳。
請於申請後翌日起以帳號或電話洽詢該警察局查詢取件期限，並於期限內攜帶應備文件前往驗證及繳費領件，逾期作廢。
郵寄申請
僅限申請人現住於海外。

應備證明文件
• 本國人：新式國民身分證正本及影本各１份或戶口名簿正本及影本各１份（持戶口名簿辦理者需另出示有相片之證件），正本驗畢歸還；需於國外的使用或登載英文姓名者，應另檢附中華民國護照基本資料頁影本１份。
。大陸港澳地區居民：中華民國臺灣地區居留證或內政部移民署核發之其他相關證件正本及影本各１份，正本驗畢歸還。
。外國人：護照或外僑居留證正本及影本各１份，正本驗畢歸還。
• 正本驗畢歸還，需加註英文姓名者，請另檢附護照基本資料頁正本或影本
• 證書費：每份新臺幣100元，同一次申請2份以上，自第2份起每份收費新臺幣20元（證明書之內容均應相同）。
• 委託他人申辦者，另檢附經申請人簽名或蓋章之委託書1份（申請書上備有委託書欄位），及受委託人身分證明文件正本。
• 申請人未滿20歲者，依據警察刑事紀錄證明核發條例第4條規定，應由其法定代理人為之。
• 現住海外人士申辦方式及應備文件：
。經我國駐外館處或財團法人海峽交流基金會或相關公證單位驗證、公證或認證之護照或相關身分證明文件影本。
。每份工本費美金7元（僅收現金），費用包含證書費及普通郵件郵資；港澳地區每份美金6元。第2份起每份收費美金1元。

。郵寄地址：10042臺北市中正區延平南路96號，臺北市政府警察局外事科收。
。地址英文：Foreign Affairs Division, Taipei City Police Department No.96, Yanping S. Rd., Zhongzheng District, Taipei City 10042, Taiwan (R.O.C.)
申請前小資訊
發證時限：一般性案件，最遲於受理申請之日起2.5個工作日發證。
經查核有尚未偵結或相關刑案資訊系統所載判決資料不完備之申請案件，則需函請司法或軍法機關查明，發證時限依個案查復情形辦理。
無法申請核發警察刑事紀錄證明情形
• 依據警察刑事紀錄證明核發條例第8條規定，申請人有下列情形之一者，不予核發警察刑事紀錄證明：
• 受通緝尚未撤銷者。
。判決確定之刑事案件尚未執行或執行中者。
• 如有前科紀錄的民眾，仍可申辦，但申請人如通緝中、假釋中、易科罰金尚未繳清或易服社會勞動尚未執行完畢，則警察機關依法不予核發。
• 警察刑事紀錄證明記載內容
• 申請人有刑事案件紀錄，除有上述情形外，受理警察機關均應核發警察刑事紀錄證明，明確記載有無刑事案件紀錄。
• 有下列各款刑事紀錄，不予記載：
。合於少年事件處理法第八十三條之一第一項規定者。
。受緩刑之宣告，未經撤銷者。
。受拘役、罰金之宣告者。
。受免刑之判決者。
。經免除其刑之執行者。
。法律已廢除其刑罰者。
。經易科罰金或依刑法第四十一條第二項之規定易服社會勞動執行完畢，五年內未再受有期徒刑以上刑之宣告者。

♦ 英文姓名的部分一定要和護照上的拼音相同，都要大寫。申請成功後就可以到選擇辦理的警察局付錢並領取良民證囉！
